Signed by Brigadier-General Zebulon Pike, 15th US Infantry. Pike was killed leading the US attack at the Battle of York, Upper Canada, on April 27, 1813, when the powder magazine at Fort York blew up. Pike had his back and ribs crushed by falling debris, and passed away after being evacuated to a ship of the invasion fleet.
